Assessing the Effectiveness of Gastric Tonometry Guided Therapy

چکیده

A low intramucosal pH (pHi) and an increase in the difference between gastric mucosal and arterial PCO2 (PCO2 gap) reflect splanchnic hypoperfusion and are good indicators of poor prognosis. Previously, some randomised controlled trials (RCTs) were performed based on the theory that normalising the low pHi or PCO2 gap could improve the outcomes of critical care patients. However, these RCTs yielded divergent results about the benefits derived from this goal-oriented therapy.

Does gastric tonometry-guided therapy reduce total mortality in critically ill patients?

Zhang and colleagues have recently published a systematic review and meta-analysis of six studies and conclude that 'gastric tonometry guided therapy can reduce total mortality in critically ill patients'. So why did gastric tonometry come and go, and what can we learn from this piece of modern history?

Gastric tonometry guided therapy in critical care patients: a systematic review and meta-analysis

INTRODUCTION The value of gastric intramucosal pH (pHi) can be calculated from the tonometrically measured partial pressure of carbon dioxide ([Formula: see text]) in the stomach and the arterial bicarbonate content. Low pHi and increase of the difference between gastric mucosal and arterial [Formula: see text] ([Formula: see text] gap) reflect splanchnic hypoperfusion and are good indicators o...
